Hockey Association (BMHA)
Registration
Please
read the registration and refund policy carefully before you register.
BMHA
has set open registration opportunities through the month of May and August
each season.
1. Early registration (Pre-Registration)
for members attending Rep Tryouts as well at New Members (Timbits) and;
These will take place
on May 4, 11, 18, 2013 from 1 – 3pm at the BMHA office,
As well as online
from May 1st to the 18th
2. Regular Registration in August for
members intending to play in our Local League Division.
These will take place
August 3, 10, 17, 24, and 31 from 12pm – 5pm at the BMHA office.
As well as on line from August 1stto the 31st
REGISTRATION:
Registration is taken on a first-come, first-served basis. Please
register early; space is limited in some age divisions. Pending lists are
available for all age divisions. Full
payment must accompany all registrations. Members who are registering for the
first time must provide a photocopy of their child’s Birth Certificate to be
considered Registered.
EASY WAYS TO REGISTER
Please
note that BMHA does not accept personal cheques.
On-Line Registration:
Atwww.bellevilleminorhockey.com On-line registration must be completed 3 days
prior to the beginning of tryouts for those who fall under the first (1) registration
period, or by Aug 31 for those that fall under the second (2).
In
Person:
1. During our
Open Registration Dates, (dates are determined each year and advertised). In the BMHA Office,
located at 265 Cannifton Rd. 1:00pm -6:00pm, Wednesdays only, during the month
of August. You may pay with cash, money order, credit card (MasterCard or
Visa) or Debit. Please provide a
photocopy of Birth Certificate if applicable.
By
Mail:
Please
include the fully completed registration form along with a Credit Card number
and expiry for the full amount of Registration, as well as a copy of your child’s
Birth Certificate if applicable. (See above)
Payment:
If
you are registering during May for Tryouts please
note that once accepted to a Representative Team you must pay the balance of
registration by June 30th of each season.
Registration fees must be paid in full at the time of registration. Payment
plans are available on an individual basis but full payment must be received
before Aug 31 of each Season. We accept cash, credit card (MasterCard or
Visa) or Debit.
Please
note that nonpayment will result in your child being withheld from the team
rosters
Confirmation:
You will receive confirmation of Registration and payment by email from the Hockey
Canada Registration Website once your Childs registration has been processed
and accepted by BMHA. All receipts are issued by email, if you require a
receipt please ensure that you provide us with your email address.
Withdrawal and Refund Policy:
1. Refunds will be granted from the earlier of a)
the date that a written request for a refund is received by the Association,
and b) the date that a player’s injury or medical condition prohibits a player
from playing. (Doctor’s letter will be required)
2. In order to be considered for a refund you are
required to complete the "Player Registration Refund Request” form. Any
requests arising as a result of an injury that prohibits a player must be
submitted within 30 days of the date of the injury.
3. Refunds will not be granted for games lost due
to injury, illness or otherwise.
A
$25 withdrawal fee will be charged per member withdrawal. Notice of
withdrawal must be made in writing and received at least 14 days before the start of the Season. Non
attendance to Games and Practice, telephone or other verbal communication is
not considered an official notice of withdrawal.
Refunds
will be processed on a percentage basis up until November 30th.
BMHA
will not process any refund requests past November 30, of each season.
Please
hang on to your receipt, there will be a $25 charge for the issue of
replacement receipts.
