Registration Information, Policy and Procedure

BMHA has 5 open registration times:
Sunday August 14 - 12:00 - 3:00PM
*Saturday August 27 - 12:00 - 3:00PM
*Sunday August 28 - 12:00 - 3:00PM
Monday August 29 - 5:30 - 8:30PM
Tuesday August 30 - 5:30 - 8:30PM
* Equipment exchange and BMHA clothing available for sizing
during these registration dates.
- Open registration for is new and returning members intending to play in our House League Division, and Timbits Program.
REGISTRATION:
Registration is taken on a first-come, first-served basis. Please register early; space is limited in some age divisions. Pending lists are available for all age divisions. Full payment must accompany all registrations. Members who are registering for the first time must provide a photocopy of their child’s Birth Certificate to be considered Registered.
FOUR EASY WAYS TO REGISTER (Please note that BMHA does not accept personal cheques):
On-Line Registration:
At www.bellevilleminorhockey.com On-line registration must be completed 3 days prior to the beginning of tryouts.
In Person:
- During our Open Registration times. Please provide a photocopy of Birth Certificate if applicable.
- You may pay with cash, money order, credit card (MasterCard or Visa) or Debit.
By Fax:
Fax: (613) 962-1041 please include your fully completed registration form along with complete credit card number and expiration date, as well as a copy of Birth Certificate if applicable (see above).
By Mail:
Please include the fully completed registration form along with a money order for the full amount of Registration, or complete credit card information, as well as a copy of your child’s Birth Certificate if applicable. (See above)
Payment:
Registration fees must be paid in full at the time of registration. We accept cash, money order, credit card (MasterCard or Visa) or Debit.
Confirmation:
You will receive confirmation of Registration and payment by email from the Hockey Canada Registration Website once your Child's registration has been processed and accepted by BMHA.
Withdrawal and Refund Policy:
REFUND POLICY
1. Any player requesting a full refund must do so before start of the 4th game. After that date partial refunds will be considered if a written request explaining the justification for such refund is received by the Board of Directors.
2. Refunds will be granted from the earlier of a) the date that a ‘ Player Registration Refund Request’ is received by the Association, and b) the date that a player’s injury or medical condition prohibits a player from playing. (Doctor’s letter will be required)
3. In order to be considered for a refund you are required to complete the "Player Registration Refund Request” form. Any requests arising as a result of an injury that prohibits a player must be submitted within 30 days of the date of the injury.
4. Refunds will not be granted for games lost due to injury, illness or otherwise.

6. BMHA will not grant refunds for games lost due to delays, deterrents or cancellations of the ice by the City as the result of: strikes, picket lines or other union actions; Acts of God; major mechanical and electrical failures which are beyond the control of the BMHA.
Please hang on to your receipt, there will be a $25 charge for the issue of replacement receipts.
